Doctors seeking accreditation at Bathurst Private Hospital can obtain a copy of the Medical Staff Bylaws & Application for Accreditation by either:

 

• Phoning the Director of Nursing on (02) 6331 7766 Ext.133 for the application form which can be forwarded to you by fax or email. 

• Email – don@bathurstprivate.com.au

 

Please forward completed application form along with the following:

 

• Copy of current medical/dental registration

• Copy of current medical indemnity insurance showing relevant specialty covered and expiry date

• Copy of CV

 

Review of Credentialing Application:

• Upon receiving your completed application, the documents will be reviewed by the DON and references will be requested from the referees

• The documentation will then be referred to the Credentialing Committee of the Medical Advisory Committee for consideration

• Upon endorsement the Medical Advisory Committee will send a letter of confirmation of accreditation at Bathurst Private Hospital to the VMO.
