PRE ADMISSION

PROCESS

Your surgeon’s rooms will provide you with a Pre-Admission booklet including health questionnaire for you to complete (sections 1, 2, 4 & 5 of the booklet).  Please see your GP if you are unsure of anything about your medical history.

 

Once completed, please forward to Bathurst Private Hospital as soon as possible either:

 

Please note that is very important that we receive your information promptly to allow us time to check your health insurance and your medical needs prior to the procedure.   The Administration staff will contact you if and advise of any outstanding fees that are payable on admission

 

The Pre Admission Nurse will call you the day before your procedure to discuss your health history and your medications.  She will confirm your arrival time and fasting requirements.  The calls are made between 110.00am – 2.00pm.  

 

 

Interpreter Services

The Translating and Interpreter Service (TIS) is available 24 hours a day and provides a free interpreting service for doctors and specialists. If you would like an interpreter at any stage of your hospitalisation, please speak with your doctor or nurse. Bathurst Private Hospital is happy to assist with these arrangements. Please be aware, there are a small number of exceptions to the provision of free services. Compensation case patients, non-permanent residents and services not booked under the doctor are not eligible for free services. In these cases, charges apply.
