PROCESS
Your surgeon’s rooms will provide you with a Pre-Admission booklet including health questionnaire for you to complete (sections 1, 2, 4 & 5 of the booklet). Please see your GP if you are unsure of anything about your medical history.
Once completed, please take this admission form out to Bathurst Private Hospital as soon as possible. You are not booked in at the hospital, until we receive this form. Alternatively, you can fax or email entire form.

Please try to provide TWO contact numbers that can reach you and at least ONE contact number for your next of kin or contact person.

You will receive a phone call from Bathurst Private Hospital Pre-admissions ONE to THREE business days prior to your procedure. You will need to write down your admission time and fasting details, so please have pen and paper at your disposal. You can call back if necessary.
You MUST be available to receive this information. Please check your phone. If you receive a message to call back, please do so as soon as possible.
Pre-admission office hours: 9:00am - 3:00pm - Monday, Tuesday and Thursday.
Interpreter Services
The Translating and Interpreter Service (TIS) is available 24 hours a day and provides a free interpreting service for doctors and specialists. If you would like an interpreter at any stage of your hospitalisation, please speak with your doctor or nurse. Bathurst Private Hospital is happy to assist with these arrangements. Please be aware, there are a small number of exceptions to the provision of free services. Compensation case patients, non-permanent residents and services not booked under the doctor are not eligible for free services. In these cases, charges apply.
